STANDARDIZED LETTER OF RECOMMENDATION (SDLOR)

This is an updated Standardized Letter of Recommendation (SDLOR) which includes a reformatted assessment section with new categories and methods of evaluating the applicant.  This PDF form can be downloaded by applicants and sent to letter writers with instructions on where to send it after completed.  It can also be downloaded by letter writers who would like to utilize it when writing letters of recommendation.

APD members are free to use this SDLOR when they are asked to write letters of recommendation.  Use of this SDLOR is not required by the APD, however, some residency programs may request letters to be submitted in this format - applicants are encouraged to check with individual programs.
